Patient Grievance & Dispute Process
If you have a concern about services, safety, quality of care, or your experience, you may submit a grievance at any time. You are not required to use a specific form.
How to submit a grievance
- Verbally: Call us and share your concern. We will document it and follow up.
- In writing: Email or mail a written statement describing the issue, relevant dates, and any details you want reviewed.
- Anonymous concerns: We will review anonymous submissions, but including contact information helps us respond.
Non-retaliation: Retaliation for filing a grievance is not permitted.
